Social Work Services
WMed has available licensed masters-level Social Workers to assist our patient population. They can assist with many items including:
Our Social Workers can assist you by helping to qualify our current patients without prescription drug coverage to get the medicines they need through a program that is right for them. The Social Workers will assist you in identifying if you qualify for prescription assistance and in completing applications for free or nearly free medicines. They will work with our pharmacists in updating medication lists and identifying resources for patients that do not qualify for prescription assistance.
Social Workers can assist you in completing a living will to be on file in case of an emergency giving your physicians instructions in the event you are unable to communicate your desire for care.

Durable Power of Attorney and Advance Directives
We all want to make our own choices about the medical treatment we receive. Sometimes we are not able to make those decisions because of a medical condition. It is important to have a plan in place to make sure that our wishes are followed. This plan is called an Advance Directive. The Advance Directive should:
- describe the type of medical care you want.
- describe the type of medical care you do not want.
- state who you want to make decisions about your care if you cannot.
- be reviewed once a year.
A Durable Power of Attorney document is used to choose a person to make decisions about your medical treatment and care.

Our Social Workers are great at uncovering resources in our community to assist in unmet needs for our patients and families.
Our Social Workers can work with our patients and other agencies to mediate issues when problems arise.
Good nutrition is a key component when treating many disease processes. WMed has a registered dietitian available to all patients as part of the multi-disciplinary team. Patients are initially seen by physician referral and follow-up is done on an individual basis as needed. She can assist our patient population in understanding food groups, weight loss and diets appropriate for their medical issues. She is also a Certified Diabetic Educator and can assist our patients in understanding how diet and diabetes go together in their daily lives.
